I declare that I provide the data voluntarily, and the personal data administrator – DS Auto limited liability company in Chrzanów provided me with the information referred to in art. 13 para. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (hereinafter referred to as ” RODO “)

Please be advised that:
1. 2. Your data will be processed in order to:
a) provision by the Administrator of the service electronically within the meaning of the provisions of the Act of 18 July 2002 on the provision of electronic services (Journal of Laws of 2017, item 1219 as amended), consisting in sending the newsletter by the Administrator – on the basis of separate consent (Article 6 (1) (a) and (GDP)),
b) conducting direct marketing – pursuant to art. 6 par. 1 lit. f RODO,
c) marketing and promotional activities – based on separate consent (Article 6 paragraph 1 letter a) and RODO),
d) sending commercial information by electronic means in accordance with art. 10 para. 2 of the Act on the provision of electronic services of July 18, 2002 (consolidated text: Journal of Laws of 2017, item 1219, as amended) – on the basis of a separate consent granted,
e) use of telecommunications terminal equipment and automated calling systems for direct marketing purposes in accordance with art. 172 of the Act of 16 July 2004 Telecommunications Law (consolidated text: Journal of Laws of 2017, item 1907 as amended) – based on the granted consent;
3. personal data will not be transferred to other recipients;
4. Your personal data will be processed until your consent is withdrawn or opposition to the processing of data (which will also occur as a result of termination of the contract for the provision of electronic services consisting in sending a newsletter) – in cases of processing your personal data on the basis of a separate consent to send the newsletter or conducting marketing activities or on the basis specified in art. 6 par. 1 lit. e or f RODO, including for direct marketing purposes;
5. providing your personal data is voluntary, but required to send a newsletter, however, failure to provide data will result in the inability to receive a newsletter from the Administrator;
6. The administrator does not process your personal data using automated decision-making tools, including profiling;
7. You have the right to request from the Administrator access to your data

personal data, rectifying, deleting, limiting the processing of such data or their transfer;
8. You have the right to object to the processing of your data, as well as the right to withdraw your consent at any time, however the withdrawal of consent does not affect the legality of the processing, which was made on the basis of consent before its withdrawal;
9. if it is found that the processing of personal data by the Administrator violates the provisions of the GDPR, you have the right to lodge a complaint to the supervisory body (ie the President of the Office for Personal Data Protection).
